
Providers may not always understand the full extent of your child's behaviors. By keeping a detailed record, you may help providers better understand your child. This logbook is intended to be a resource for the child's family. It is NOT intended to be used as a disciplinary method for the child's behaviors. Keeping a detailed record aids caregivers in communicating information to providers. The organizational flow of this book allows caregivers to refer to specific dates, times, and events easily. Share with providers to: help in proper diagnoses help determine if new medications or methods are working help them better understand the severity/frequency of incidents help them refer your child to the proper services/care help them formulate the most effective treatment plan help them better meet the needs of your child and your family Useful for logging: Medication refusals School or therapy refusals Incidents of aggression/violence Incidents of property destruction Threats Incidents of severe anxiety/anxiety attacks Incidents of depression/withdrawal Mental health-related emergency room visits Inpatient hospitalizations In addition to logging incidents, the Incident Log for Caretakers of Children with Mental Health or Behavioral Disorders includes areas to record your child's current providers as well as medications, and diagnoses.
