This report contains the findings of the Phase II audit includingrecommendations for changes to the APIOS programme. The firstchapter summarizes the audit results and points out majorstrengths and weaknesses of the programme. The next chapter isdedicated to performance related audit findings. The thirdchapter reports on system related audit findings, giving detailson modifications recommended to network, quality assurance andefficiency aspects of the programme. The last chapter collatesthe recommendations in summary form, and reviews the actions onthe recommendations from the Phase I audit. Volume II of thePhase II Audit Report contains the completed Phase II site auditquestionnaires prepared by CSC's auditors, including photographsof the audited sites.
